STOP with CHCs and AHACs
Following an invitation from the Ministry of Health and Long-Term care in November 2011, the first CHCs began implementing the STOP with CHCs program in February, 2012. The goals of this program are to:
1) Improve access to nicotine replacement therapy for CHC patient populations across Ontario who wish to quit smoking; and
2) Integrate smoking cessation programs into CHC models of care to provide accessible cessation interventions and medications to these patient populations.
Collaboratively, STOP staff and CHC staff are working together to identify implementation best practices to address the specific requirements of each community’s patient population.
STOP provides participating CHCs with Nicotine Replacement Therapy (NRT) for enrolled patients, along with ongoing training and knowledge exchange opportunities to encourage practitioners to incorporate up-to-date smoking cessation treatment approaches into their routine practice.
Get Involved: Information for individuals who wish to quit smoking
Who can participate: The Community Health Centres offering the program reach out to participants in a variety of ways. There may be eligibility requirements for enrolment at your Community Health Centre. This program is only open to residents of Ontario who currently smoke cigarettes.
When: Enrolment times vary and are determined by each participating Community Health Centre based on their capacity.
If you are a patient or client at a Community Health Centre in Ontario, you can contact your Community Health Centre to find out if they are a STOP Program partner currently enrolling eligible participants.
