Implemented in July 2011, The STOP with Family Health Teams (FHTs) program has two goals:
1) to improve access to nicotine replacement therapy for FHT patients across the province of Ontario who wish to quit smoking; and
2) to increase capacity of health care practitioners in FHTs to provide comprehensive smoking cessation treatment to their patients
Family Health Teams participating in the STOP with FHTs program are provided with free Nicotine Replacement Therapy (NRT) for enrolled patients, along with ongoing training and knowledge exchange opportunities to encourage practitioners to incorporate up-to-date smoking cessation treatment approaches into their routine practice.

Who can enroll: STOP with FHTs program is designed to reach patients who are already part of participating Family Health Teams in Ontario. Family Health Teams may have additional eligibility requirements for enrollment.
When: Enrollment times vary and are determined by each participating Family Health Team based on their capacity.
If you are currently a patient at a Family Health Team in Ontario, please contact your health care provider(s) to find out if your Family Health Team is currently enrolling eligible participants into the STOP with FHTs program.
Please note: The STOP with FHTs program is currently available only to patients of participating Ontario Family Health Teams. If you are unsure whether you belong to a Family Health Team, please contact your family doctor.
